ARREARS OF WIDOW'S PENSION. Act of June 7, 1888 Allow arrears of pension at the rate of $8.00 per month from July 16, 1885, and two dollars a month additional for each child, as follows: LeRoy C. Born March 13, 1871/Comm. July 16, 1885 and ending Sept. 10, 1885 when original pension commenced. Was pensioned April 5th 1886 at $8.00 per month from September 11, 1885 and $12.00 per month from March 19, 1886 and two dollars a month additional for each child as above stated. EXAMINING SURGEON'S CERTIFICATE No. of Application 341726 I hereby certify that I have carefully examined Lot H. Carley, late a private Co. E, 5 Regiment, Iowa Vols. in the service of the United States, who is an APPLICANT for an invalid pension by reason of alleged disability resulting from scurvy & chronic diarrhea. In my opinion the said Lot H. Carley is 7/8 total incapacitated for obtaining his subsistence by manual labor from the cause above stated. Judging from his present condition and from the evidence before me it is my belief that the said disability did originate in the service aforesaid in the line of duty. The disability is permanent. A more particular description of the applicant's condition is subjoined: Height, 6; weight, 147; complexion, dark; age, 41; pulse, 78; respiration, 17 Mucus membrane of mouth dark red, spongy and bleed very easily. Gums spongy and fallen away from teeth, teeth loose and black, breath offensive - Disability 3/8 total. Abdomen tympanitic and tender and pressure. Has from 3 to 8 muce-purulent, and occasionally bloody stools daily for several days to be followed by obstinate constipation for a few days then alternate with diarrhea again. Disability 1/2 total, General health bad. Claimant says the above disabilities were contracted while in Andersonville Prison Cephas L. Carroll {Diagram} Surgeon's Certificate In Case of Application for Pension EXAMINING SURGEON'S CERTIFICATE No. 341,726 Examining Surgeon's Address: Date of examination, Dec. 20th, 1882 (This document was copied with a piece of paper laying over a portion of the contents, I am unable to type the full contents of the document.) ____I hereby certify that we have carefully examined this applicant, who claims that while in the service . . . place named Andersonville . . . on or about the -- day of July . . . Ch. diarrhea & scurvy . . . in consequence thereof he is 3/8 disabled for earning his subsistence. . . He states that he is 42 years of age, that he . . . pounds, and that he is 6 feet -- inches. . . His pulse-rate is 75 . . . his temperature normal. The examination reveals the following. . . Phys. cond. ratherfuble. . . Contracted diarrhea . . . Andersonville and two other . . . after his discharge. Ever since . . . unable to perform more than half a mans. . . no signs of any . . . disease . . . some absorption gums, as if he had suffered from scurvy and an appearance of delicate constitution. We give credit to his statements and believe his total powers have been impaired by the privations suffered while in prisons. If statements are proven, Dis. 3/8 From the condition and history of the claimant, it is our opinion the disability was truly incurred in the service as claimed, and that it is no aggravated or protracted by vicious habits. State of Kansas, Margaret A. Carley, of Wamego, being first duly sworn, declares and says that she is the widow of Lot H. Carley a Sergeant of Company E, Fifth Iowa Volunteer Cavalry; that she receives a pension under Certificate No. 221,434; that she was born January 20, 1845, and is, therefore, entitled to an increase of pension under the present pension law; that she was married to the said soldier February 18, 1864. And further affront saith not.
I hereby certify that during the year of 1882, I prescribed for Lot H. Carley, late of Co. E 5th Iowa Cavalry, for general debility and I further certify that I have no interest whatever in the prosecution of his claim for pension. G.M. Staples Witness my hand and notary seal. I, James J. Medical Evidence PHYSICIAN'S AFFIDAVIT State of Iowa, County of Dubuque, SS. Personally came before me, a Notary Public in and for the aforesaid County and State, E. B. Jackson, M.D. a citizen of Dubuque County whose Post Office address is #855 Salust Street Dubuque Iowa, well known to me to be reputable and entitled to credit, and who, being duly sworn declares in relation to aforesaid case as follows: That he is a Practicing Physician, and that he has been acquainted with said soldier for about 11 years, and that did not know claimant prior to enlistment. First prescribed for him about May 1883 and at different times from that time until Apr 18th 1884 when I last prescribed for him. His difficulties were general debility and disange such of that stomach shown by pain after eating, acidity and at times vomiting. His strength was so impaired that he would give out and be compelled to lie down for a time whenever he was -- at other times could perhaps work the greater part of the day. He further declares that he has been a practitioner of medicine for eleven years, and that he has no interest either direct or indirect, in the prosecution of this claim. E. R. Jackson, M.D.

He was sick in January 8? suffering with chronic gastritis. He was very weak when I first got acquainted with him in September 84. He was emaciated, had no constitution left to build upon, was anaemic and suffered from general debility caused by army and army foods. . . On the 19th day of June he took sick again with gastor intestinal lesion; main symptom being vomiting. He gradually got weaker as no nourishment could be retained on his stomach. The general debility as after effect of army and prison life in Andersonville caused inanition and loss of vital forces. His death took place July 15th, 1885, asthma being the immediate cause. From January to June he was unable to . . . I also state, that I am not interested directly nor indirectly in the prosecution of this claim. Dr. A. Oppermann In the presence of E. Oppermann aged 43 years, a resident of North Auburn Nemaha County Nebraska whose post office address in North Auburn, Nemaha county Nebraska well know to me to be respectable and entitled to credit and who being by me first duly sworn declares in relation to the aforesaid cause as follows: Office of Dr. A. Oppermann, Physician & Surgeon No. Auburn, Neb. Decbr.7th 1885 To whom it may concern: This is to certify that I professionally treated Mr. Lot H. Carley in his last illness. In January 1885 he was sick, chronic invalidism, caused by general debility, as after effect of army and especially prison life. He regained partially his strength until June 6th, 1885, when he again fell sick with the same chronic debility, loss of vital power by inanition, caused by army prison life. He gradually sank and died on Wednesday, July 15th, 1885. This is a true statement, given under my hand and seal this the 7th day of December 1885. Dr. A. Auburn, Nebraska, June 15th 1888 To the Comm. of Pensions, I see in the Solids Blade of June 14th that the amendment to the Bill pensioning soldiers widows has become a law. And that all that is necessary for such widows as shall come under this law of claims is to send marriage, &c., the numbers of my certificate is 221.434. Margaret A.
